This program assists patients to obtain brand-name prescription medications only - not over-the-counter products or generic prescription medications.
What are the guidelines to qualify for company programs?
Each company has its own specific guidelines, forms and criteria. Generally, a patient must show financial need and also that they have no other resource for helping with the cost of prescription medications.
How do I show financial need?
You must provide documentation of all income. For example, the average maximum income to qualify is $16,000 per year per individual and $25,000 for a household of two. There are some exceptions with some companies.
Examples of acceptable documentation include:
Copies of income documentation, such as your Social Security Benefit Statement for the current year, paycheck stubs, unemployment payment documentation, pensions, IRAs, interest on investments and savings. If taxes are filed, a copy will be required each year.
List of medical expenses may include non-prescription medicines or supplies, doctor and dental bills, and eye care.
Must I show that I have no other resource for helping with the cost of prescription medications?
Sometimes you must provide documentation of denial of coverage from an insurance company or State or Federal program. This can be done by applying and receiving a denial form.
Can another person help me with forms and documentation?
Yes, if you need assistance due to issues with hearing, eyesight, or reading and writing of English, please inform us of a contact person including their name, phone, address and relationship to you.
What is the typical process for qualification and approval?
Each company has a different process. We will let you know about the specific company process once it is determined which program you will be applying for. Company approvals are for limited times, usually from three months to a year, and the process will be repeated for further assistance.
What should I expect to happen after my application has been made?
The time between our first interview with you and delivery of medications can be as long as two to three months. It may take longer if complete information is not provided. Once the company receives your application, they perform their review. Some companies inform the patient of acceptance by letter, others include an ID card or they issue coupons, and some charge a minimal copayment. In some cases, medication may be delivered to your home, requiring a signature on delivery. Others simply send your medications to your physician's office and the office should notify you once they arrive.
Will I need to re-apply?
For all company programs, you will need to re-apply periodically, sometimes as often as every two months depending on that company's procedures. This may involve completing and signing application forms again and providing updated documentation of financial and medical information. We will help you through the process.
Is this a complete, permanent solution to my medication needs?
Unfortunately, the program cannot provide a complete or permanent solution for you. It will always be a good idea to try to find more permanent coverage such as qualifying for state or federal programs, or private insurance.
