How to enroll

PEBB members can conveniently enroll online for Providence Choice Plan benefits during open enrollment.

Download enrollment forms at http://oregon.gov/DAS/PEBB/forms.shtml. These forms are processed by PEBB and must be submitted through your benefits administrator.

If you would like to contact PEBB, you may:

• Call 503-373-1102 (Salem number).
• Call toll-free 1-800-788-0520 (in-state calls only).
• Visit the PEBB Web site at http://www.oregon.gov/das/pebb.

If you are currently a Providence Choice Plan member, you do not need to enroll. Your membership will automatically continue with Providence Choice Plan.
