Notice of Privacy Practices

Emilie Court Assisted Living (EC) is committed to protecting the confidentiality of your medical information and is required by law to do so.  This Notice of Privacy Practices describes how we may use and disclose your protected health information to carry out treatment, payment, or health care operations, and for other purposes that are permitted or required by law.  It also describes your rights to access and control your protected health information.  
 
Uses and Disclosures
Emilie Court Assisted Living may use or disclose your protected health information as follows:

For Treatment:  Emilie Court may disclose all or any portion of your medical record to your attending physician, consulting physician(s), nurses, technicians, medical students and other healthcare personnel who are involved in your care. Your physician may also share your health care information with other physicians in their practice.

For Payment:  Emilie Court may use and disclose medical information about you for the purpose of determining coverage, billing, claims management, medical data processing, and reimbursement. The information may be released to an insurance company or a third party payer or other entity (or authorized representative) involved in the payment of your medical bill upon your prior authorization obtained on the “Residency Service Agreement” form.
 
For Health Care Operations: Emilie Court may use and disclose medical information about you in order to support quality improvement and other business activities of our organization.  These uses and disclosures are necessary for Emilie Court operations and ensure the quality of care received by our residents. 
 
Facility Directory: In order to assist family members and visitors while you are at Emilie Court Assisted Living, a facility directory is maintained. If you would like to opt out of being included in the facility directory, please make this request to the Director or designee. 
 
Your Rights Regarding Your Medical Information:

• You have the following rights regarding your protected health information:
  • Right to inspect and copy
  • Right to amend
  • Right to request restrictions
  • Right to an accounting of certain disclosures
  • Right to request confidential communications
  • Right to a paper copy of this notice

Complaints
If you believe your privacy rights have been violated, you may file a complaint with the Compliance Office at Emilie Court Assisted Living, 34 E. 8th Avenue, Spokane, WA 99202-1202 or call (509) 474-2550.  You may also file a complaint with the Secretary of the Department of Health and Human Services.  You will not be penalized for filing a complaint.
