  • Adult residents of Thurston and Mason counties 18 years of age and older
  • Individuals who are homebound because of a physical disability, medical or mental health problem
  • Adults who are challenged by acute, serious and/or persistent mental health problems

  • Mental and physical evaluation
  • Medication management and education
  • Individual and family counseling
  • Training in independent living by licensed occupational therapists
  • Education and support for families
  • Coordination of mental health treatment with other medical needs
  • Coordination of services for skilled medical and surgical nursing care and home health aides
  • Eligibility screening and linkage to community resources and advocacy

Team members
Our highly qualified home mental health team includes a psychiatrist; registered nurses skilled in psychiatry, geriatrics and medical care; occupational therapists; physical therapists; social worker, master's level therapist; home health aide; and speech therapist.

Payment
Services are reimbursed through Medicare, Medicaid, personal insurance and private pay. Financial counseling and coordination with insurance companies are available.
