What is Home Care?
Home health care is skilled, personalized service provided to patients in their homes, including assisted care and adult family home facilities. Home health team members serve the needs of patients recovering from illnesses, accidents or surgery, and provide assistance to those living with a disability or chronic illness.

- Registered and licensed nurses who provide skilled nursing care according to a plan of care developed for each patient with his or her physician.
- Certified nursing assistants who help with personal care and other activities of daily life.
- Physical, occupational and speech therapists who work to increase patient strength, mobility, safety, and communication skills.
- Medical social workers who identify resources for patients and help them with counseling, problem solving, and other support.

Insurance benefits can be confusing in today's health care environment. Our knowledgeable staff will coordinate with you, your physician and your insurance provider to determine your insurance benefits.
