 Fill out a Pledge Form
Simply complete the pledge form with the dollar amount to be deducted, the total amount of your gift and your signature to authorize payroll deduction. Payroll deductions for your contribution will start in January 2008.
Providence employees may return the pledge form to the Foundation office at Providence St. Peter Hospital (second floor past the cafeteria - mailstop 02H24). Be sure to keep a copy of your completed form for your records. Please direct any questions to the Foundation office at 3-7981.
