Most therapy services require a physician referral. We work with you and your third party payer and are a provider with most insurance carriers. Our program criteria and admission/referral information for different programs and services are as follows:
Comprehensive Integrated Inpatient Rehabilitation Program (CIIRP)
The Acute Inpatient Medical Rehabilitation unit is adjacent to the main hospital and provides 24-hour-a-day rehabilitation nursing, physician care coordination, physical, occupational, speech/swallowing therapy, cognitive rehabilitation, psychology, recreational therapy and other services as needed. Patients have access to full hospital and physician services. This program is accredited by CARF (a national accreditation program for medical rehabilitation programs) and is also a Level II trauma rehabilitation provider (Washington Department of Health).
Referrals: Phone: (360) 493-7640; Fax (360) 493-4457 or contact cindy.duke@providence.org.
Our program requires a pre-admission screening to be sure this is the appropriate level of service. We work with payers for appropriate pre-authorization. We welcome inquiries from other healthcare settings including acute hospitals, skilled nursing facilities, home health but ask for a physician referral for us to screen a patient at these settings.
