To meet your need for continuing care, members of our staff will assist you, your physician and your family to continue your care after you leave our hospital. Some of these services include:
Durable medical equipment companies provide a wide range of medical equipment supplies and oxygen therapy equipment to patients in their homes. This is facilitated through Case Managers. Ask your nurse for additional information.
Full service Medicare-certified home care agencies supply nurses/nurse aides, medical social workers and therapists for home health care. Private duty nurses, nurse aides and adult sitter services for discharged patients are also available. Ask your nurse for additional information.
