
The Diabetes Care Center at Providence Tarzana Medical Center is an American Diabetes Association Recognized Program – one of only three in the San Fernando Valley. The Center is located across the street from the medical center at 18344 Clark Street, Suite 210 in Tarzana. Our multi-disciplinary diabetes care team consists of: Specialty Physicians: Cardiologists, Diabetologists, Nephrologists, Neurologists, Opthalmologists and Podiatrists Registered nurses who are certified diabetes educators Registered dietitians who are certified diabetes educators Mental health professionals
The Diabetes Care Center offers a wide variety of inpatient and outpatient services to help patients of all ages with Type 1, Type 2, gestational diabetes and other forms of impaired glucose metabolism – referred to as pre-diabetes. With access to the most current information and technology regarding diabetes care, these diabetes experts provide appropriated education to both patients and their families about all aspects of diabetes care, which include: Nutrition and meal planning Exercise benefits and precautions Medication instruction – as needed Lifestyle adjustments Insulin administration Foot care Coping with the stress of diabetes Educating patients with pre-diabetic conditions in order to prevent or delay the full onset of diabetes
