The High Risk Infant Follow-up Program at Providence Tarzana Medical Center provides diagnostic services for children who are at high risk for neuro-developmental concerns between 3 and 36 months of age. The program is available to all infants who meet certain criteria as determined by the NICU
medical team that cared for them, or are referred by their pediatrician.
After babies have graduated from the NICU, a special team of health care professionals tracks their growth and development. The team provides comfort and support to parents and caregivers by addressing their concerns and expectations.
Our team of specialists includes:

Provides a comprehensive diagnostic assessment to identify potential developmental and medical issues as early
as possible.
- Michelle Schaubert, Pediatric RN/Clinic Coordinator
Coordinates overall operation of the clinic, facilitates team conferences and communicates results, recommendations and referrals to the appropriate individuals or agencies.
- Debbie Solomin, RD, MPH, CDE, Dietitian
Assesses nutrient intake and growth. Provides education and materials on age appropriate healthy eating, individualized for each child.
- Leilah Sadd, MSW, LCSW, Social Worker
Assesses psychosocial needs, parenting techniques and provides emotional support and information about appropriate community resources.
- Astri Sorgaard Hilton, Physical Therapist
Christy Hennessey, Occupational Therapist
Provides comprehensive developmental assessment to evaluate milestones. Assists team and parents with family educations and provides appropriate referrals as needed.

Appointments
The first appointment at the High Risk Infant Follow-up Program is usually scheduled for the baby when he/she will be 6 months old (corrected age for prematurity). Subsequent visits are scheduled for children when they are 12–14 months old and 18–24 months old. Appointments may take up to
1–1 1/2 hours.
During each visit, the neonatologist, pediatric nurse, social worker, dietitian and physical therapists will thoroughly review your child’s progress by asking questions, performing a physical exam and administering a developmental test. The developmental test will assess your child’s motor skills, comprehension and language skills.
The team will discuss the child's evaluation with parents and caregivers, answer their questions and concerns, and provide appropriate referrals as needed. The child's pediatrician will receive a written report about your child's progress after
each visit.
