How to print out a Delta Dental ID Card
Click on the Subscribers box on the upper left side Click on Subscriber Connection New Users must Register in (click on Register Here) Complete the required information for login Once complete, you will be logged in to your own personal page To print an ID Card, scroll down and click on ID Card Click on the name that you need a Dental Card for. Two cards that are personalized will appear, click on Printable Version Print out the cards and use as your Dental Card.
