How to access ProvCare clinical results: Physicians (Radia physicians see below) The following forms will be required unless you have already completed them: Step 1. Physicians who wish to sign up for a OneHealthPort (OHP) account should first register your practice with OHP and choose a OHP Administrator (typically your office manager). Go to the OHP organization registration page. Step 2. Once your practice or healthcare organization has registered with OHP, you must be "nominated" by your OHP Administrator. Step 3. Upon nomination OHP will send your User ID directly to you. OHP will send your activation code to your OHP Administrator. You will need both your User ID and your activation code to complete the online subscriber registration process. Go to the OHP registration page to complete this process. Then complete Step 4. Step 4. You must return to this Web page to request a hardware ID. Once you are registered with OHP and have your User ID, submit this form to request a hardware ID (if your browser's popup blocker is turned on, you will need to turn it off).
