About Darius W. Graeff
As both a primary care provider and a sports medicine specialist, Dr. Graeff hopes to have a tangible, positive impact on the people around them. “I chose family medicine to empower patients to make informed decisions, to navigate the world of medicine, and to feel in charge of their own health,” they say. Their added specialization in sports medicine broadens the scope of care that they can provide, from helping patients optimize function and prevent illness through nutrition and exercise, to managing athletic injuries, concussions and fractures. They also have extensive training in ultrasound diagnostic procedures and in ultrasound-guided therapies to help relieve arthritis, bursitis and other joint symptoms.
Dr. Graeff is passionate about caring for members of the LGBTQ+ community, including providing comprehensive care for transgender and non-binary individuals. “I believe in respecting every patient’s thoughts, feelings and individuality,” they say. “My most important role, with every patient, is to provide an environment of warmth, safety and understanding.”
Born and raised in Upstate New York, Dr. Graeff completed most of their education in their home state, earning degrees from the University at Albany and the New York Institute of Technology College of Osteopathic Medicine, and completing a family practice internship and residency at Northwell Plainview Hospital. Their sports medicine fellowship took them to The University of Tennessee Graduate School of Medicine, where they provided medical coverage for the rugby, football and hockey teams. While there, they also served as the team doctor for Team USA’s junior weight-lifting team during the 2021 Pan American Games. Dr. Graeff is board certified in family medicine and sports medicine.
In their free time, they stay active with weight lifting, running, yoga, hiking, camping, and covering sports events as a physician. They also enjoy reading and exploring local coffee shops in hope of finding the best cold brew in town.

I had a great visit with Dr. Graef. He never mentioned my weight, which I was expecting and not looking forward to, however I was met with respect and felt so comfortable with this provider. I have a lot of anxiety with healthcare and tend to put my care off. I wish I'd known sooner what an exceptional care I would have, I would have come in sooner and not let myself go through so much pain. I would wholeheartedly recommend Dr. Great, oh I mean Dr. Graef.
