We serve the greater Southern California region including Los Angeles County, Inland Empire, San Bernardino, Orange County, Riverside and San Fernando Valley from our Burbank-based Home Infusion Pharmacy. Our goal is to help make your infusion therapy go comfortably and smoothly. Your clinical team includes clinical pharmacists, registered nurses, pharmacy technicians and registered dietitians. Infusion pharmacy

Clinical pharmacists work closely with the patient’s physician and the infusion nurse and dietitian to coordinate care for all prescribed therapies. Our pharmacists have specialized experience working with patients in the areas of nutrition, infectious diseases, oncology, specialty iv drugs and pain management.

Infusion nursing services

We complete a thorough patient assessment prior to initiating infusion therapy at home to ensure that a patient and family will be successful with home infusion services. Infusion nurses provide one-on-one training with patients and caregivers so they can self-administer infusion therapies and manage pumps and other specialized infusion equipment. Some intermittent specialty* IV medications require our nurses to administer medications and monitor patients during infusions. Our infusion nurses are available to the patient or caregiver at any time by phone should assistance be needed.

Registered dietitian services

Patients receiving parenteral nutrition (IV Nutrition) and enteral nutrition (tube feeding) are regularly assessed and monitored by our registered dietitian, who is available to consult with patients on therapy tolerance and transitional feedings. Dietitians make recommendations for therapy changes based on nutrition assessments in coordination with our infusion pharmacists and patient physicians. Dietitians are also available for referral consulting with referring providers.

In investigating home IV therapy, it is important to understand that you, and your care partner, will need to be able and willing to perform the steps needed to administer your IV therapy at home. A home infusion-trained nurse will teach you and a care partner to administer your therapy as ordered by your physician. After teaching, a nurse will see you weekly for IV line dressing changes and lab draws. For your support with urgent infusion matters after business hours, our pharmacy has a pharmacist on call 24/7. Providence Home Infusion Pharmacy provides home infusion services in the greater Southern California area and can coordinate patient transfers to anywhere in the United States. We have sister home infusion pharmacies in Oregon and Washington as well.

In addition to being a non-profit provider, St. Joseph Home Infusion Pharmacy - Anaheim is fully accredited by The Joint Commission and State Licensed Pharmacy.

We are contracted with most major insurance providers. Please phone our Southern California intake team to confirm we are able to serve you through your insurance plan. Our intake coordinators will get back to you promptly and let you know whether home infusion care is covered under your policy. If we are not contracted we will let you know who is.

Providence Home Infusion Pharmacy provides home-based infusion therapies to the greater Southern California area and can coordinate nursing services and facilitate patient transfers to anywhere in the US. In addition to being a non-profit provider, our infusion pharmacy is fully accredited by The Joint Commission, is a State Licensed Pharmacy and works in collaboration with our State Licensed Home Health Agency partners.

Providence Home Infusion Pharmacy is contracted with most major insurance payors. You can always call our intake team, 818-847-6558, to ask one of our intake coordinators whether we are contracted with an insurance payor.

Providence clinical liaisons

The Providence Clinical Liaisons and intake team's objective is to facilitate the IV therapy referral and admission process. Your liaison facilitates IV therapies according to your facility’s needs. Some of the services they provide are:

  • Receiving initial referral calls and fielding questions
  • Obtaining referral information for the Providence Home Infusion intake team
  • Introducing services to patients choosing Provience Infusion as their home IV provider
  • Meeting with hospitalized patients and care partners to answer questions
  • Coordinating start of services with intake, pharmacy and nursing teams
  • Ongoing communication with discharge planners and care mangers from when referral is received to patient discharge
  • Working with physician offices and specialty clinics to facilitate office-based referrals
Billing and insurance

Services provided by our network will be billed to your insurance. We will obtain necessary authorizations from your insurance and physicians to determine home health care coverage eligibility. We will provide you with a financial responsibility letter outlining your insurance benefits and cost specific to the services rendered by our agency. This will help you make an informed decision about your care.

For patients with Medicare, Medi-Cal or private insurance, we will bill your insurance directly. Patient responsibility, identified by your insurance carrier, will be billed to you on a monthly basis. For patients that do not have insurance coverage, we will bill you or your guarantor directly for the services rendered. As the patient, you are ultimately responsible for payment for the services rendered.

Interpreter services

We have multilingual clinicians to meet a variety of languages represented in the community we serve. In addition, we provide telephone interpreter services through Pacific Interpreters. Call 818-847-6558 and we will assist you in finding the right care.

Patient safety and quality care are of paramount concern at Providence Home and Community Care. We have implemented a number of strategies to achieve continuous improvement in this area. Our patient safety officer combats safety challenges by developing safer procedures and educating our staff about these improved methods.
