Attention Commercial Drivers
The Federal Motor Carrier Safety Administration requires all interstate commercial driver medical examinations be performed by medical providers listed on the National Registry of Certified Medical Examiners.
All Providence Occupational Health providers meet these requirements and can sign your medical card. Callus today at 406-329-5746 to schedule your appointment.
f your employer requires you receive a medical exam through our Occupational Health clinic, we won’t need insurance information from you. We will bill your employer directly. If your exam is related to a workers compensation claim, please be prepared to provide your claim number when you check in.
Our clinic is in Suite C of the Garden City Medical Arts building, located behind the former Safeway building and across the street from St. Patrick Hospital. The clinic is at the west end of the building.
If you need lab work, you may need to fast for 12 hours before your appointment. Please call 406-329-5746 to find out if this applies to you.
