St. Joseph Health Home Health, a division of Providence Home and Community Care, has been providing medical care in patient’s home since 1988. Serving all of Orange County, our team of expert clinicians bring personalized medical care into the home.
Eligibility for home health services is dependent on your physician's order and your insurance provider. Our intake department can help you determine eligibility. Call our intake department at 714-712-7110. We will assist you in finding the right care.
For patients who have insurance, the coverage will vary depending on each individual plan. For patients who have Medicare or Medi-Cal, coordination of benefits is arranged through our agency. Did you know you can make your own referral? Referrals can also come from hospitals, physicians, insurance case managers and families. For more information or to initiate the referral process, please call 714-712-7110..
Making a referral for you, a family member or other loved ones is as simple as filling out the form below. Someone will respond to your referral to discuss the care needed and offer next steps to receiving care.
Billing and insurance
Services provided by our network will be billed to your insurance. We will obtain necessary authorizations from your insurance and physicians to determine home health care coverage eligibility. We will provide you with a financial responsibility letter outlining your insurance benefits and cost specific to the services rendered by our agency. This will help you make an informed decision about your care.
For patients with Medicare, Medi-Cal and or Private Insurance, we will bill your insurance directly. Patient responsibility, identified by your insurance carrier, will be billed to you on a monthly basis. For patients that do not have insurance coverage, we will bill you or your guarantor directly for the services rendered. As the patient, you are ultimately responsible for payment for the services rendered.
We have multilingual clinicians to meet a variety of languages represented in the community we serve. In addition, we provide telephone interpreter services through Pacific Interpreters. Call our intake department at 714-712-7110 and we will assist you in finding the right care.
Quality and safety
Patient safety and quality care are of paramount concern at Providence Home and Community Care. We have implemented a number of strategies to achieve continuous improvement in this area. Our patient safety officer combats safety challenges by developing safer procedures and educating our staff about these improved methods. In addition, we encourage our patients to participate in our efforts to increase safe practices in their home.
