The home care and hospice teams work together with physicians, patients, family members and insurance providers to meet each patient's unique needs. Access to our professional services is available 24 hours a day, every day. Whether you or a loved one needs short-term rehabilitation on the journey to wellness, or palliative and hospice care to increase comfort near the end of life, we are here for you. Serving Mason, Lewis, Pierce and Thurston counties, Providence SoundHomeCare and Hospice is a local, non-profit, state licensed and Medicare certified agency.

The onset of a chronic or terminal disease can cause great turmoil within families. Emotions become intense and stress levels rise. Patients and their families not only have to cope with new medical diagnoses, but also with the financial, social and psychological stresses that accompany them.

Medical social workers are here to help. Social workers work with patients and their families to manage these new stresses and obtain the services and support necessary to surmount them.

  • Assessment and evaluation: Social workers visit with patients and their families and help them review their situations. Available resources are discussed and help provided to access them.
  • Counseling: Social workers help patients and families deal with the emotions evoked by the onset of chronic and terminal illnesses. Social Workers help families with problem solving, education, family counseling, support and crisis intervention.
  • Community resource planning: Social workers provide information about community programs and resources to help patients and their families cope with chronic and terminal illnesses.

We serve home health patients in Western Washington and in the Spokane region and we serve hospice patients in Western Washington. We promise to respond to all hospice inquiries within one hour.

We accept payment from Medicare, Medicaid, private pay and most insurance plans. As a courtesy, we will bill your insurance company.

Financial assistance for our services is available for those who qualify. Please call us if you have additional questions about accepted health insurance plans or financial aid.

There are many things caregivers can do in the home to look after their loved ones and assist with their daily needs. From helping the patient transfer from bed to a wheelchair to repositioning the patient in bed, friends and family members can contribute to the patient's care.
