Outpatient lab services consolidated to Chase Medical Complex

Beginning May 1, 2021, Providence Medical Group is consolidating its outpatient lab services to one location -- the Chase Medical Complex, 380 Chase Ave. Lab draws will no longer be done at the Providence Southgate Medical Park.  The Chase lab will be open week days from 7 am – 5 pm, and Saturdays from 8 am – 12 pm. On Saturdays, please enter the Chase Medical Complex through the door marked “Internal Medicine”.
