With its partner, the Prevention Institute, SJCPF brings together cohorts from the communities served by the Providence St. Joseph Health System hospitals to advance health equity by addressing the community conditions that shape health outcomes. The partners have been working for several years to identify challenges and opportunities for hospitals and their partners to address the upstream causes of community-level health problems.
