TrinityCare Hospice

Founded in 1977 as a division of the Hospital Home Health Care Agency of California, Providence TrinityCare Hospice began as a small, local team that has since evolved into a vibrant regional program with five clinical teams and a staff of more than 180 dedicated professionals.

An innovator and leader in providing hospice care for patients and families in need of end-of-life services, Providence TrinityCare Hospice was one of a select group of hospices in the nation chosen to participate in a demonstration project through the U. S. Department of Health and Human Services. The project assisted the federal government with the legislative proposal that made hospice a Medicare benefit to all beneficiaries. Years later, Hospital Home Health received the first hospice license issued by the State of California in recognition of the organization's efforts in lobbying for hospice state licensure.

Providence TrinityCare Hospice has received special commendations in 2002, 2005 and again in 2008 from the organization's accrediting body, Community Health Accreditation Program (CHAP), in recognition of the development of innovative programs, excellence of community health care practice and quality performance.

Providence TrinityCare Hospice provides a full range of palliative care, hospice care and compassionate support to our patients and their families throughout our Southern California Medical Centers. By embracing a holistic and family-centered approach to hospice care, we work tirelessly to provide for your every physical, emotional, social and spiritual care.

All of our care is provided wherever our patients and their families need us; in hospitals, nursing homes, rehabilitation facilities, and in the patient's own homes. Our services are tailored to each individual, whatever is needed.

When calling the Torrance Team:
Calls made Monday through Friday from 8:00 am to 5:00 pm will be answered first by an automated attendant. Select the option for the Torrance Adult Team or press "0" for the receptionist. When the receptionist gets on, ask her or him for the Torrance Team Clinician or Clinical Supervisor.

When you call Monday through Friday from 8:00 am to 5:00 pm:
Your call will reach the team office. Ask to speak with a Hospice Clinician or the Clinical Supervisor who will assist you.

When you call after 5:00 pm on weekdays, or anytime on weekends or holidays:
The answering service operator will answer the phone. The answering service operator is not a trained hospice professional and will not be able to assist with providing medical advice or answer questions. You should ask the answering service operator for the Hospice Nurse and the operator will immediately page the Hospice Nurse for you.

You can now relax. The Hospice Nurse will call you back. He or she will have access to information about your loved one's condition, medicines, physician's name and phone number, and other pertinent information. The Hospice Nurse will assess the problem, consult with other team members, answer your questions and explain changes that your loved one may be experiencing. The Hospice Nurse will also provide support and advice and arrange for a Hospice Clinician to visit if needed.
