My husband and I are both very pleased to have Dr Chan assigned to us as our PCP. She is knowledgeable, personable and thorough. We are not pleased about the upcoming closure of the Hawks Prairie Immediate Care clinic.

Both staff and doctor were so, so good. I had trouble getting an appointment that wasn't months later, but that's the situation everywhere. Doctor had to listen to a long list of chronic conditions and is the first to not react as if overwhelmed. Instead, she remarked that I have a lot of conditions and started making a plan. She is SUPER smart and calm. Very good experience overall. The nurse (phlebotomist?) that took my blood was excellent as well, very experienced so I was surprised to hardly have any pain.

All good every time. Always feel better informed after the visit. The doctor and her staff are my A Team.

Very good first visit to establish care with Dr. Chan. My previous Providence PCP retired in early January, and I made appointment in December to see Dr. Chan. I would have preferred to establish care with next doctor sooner, but I had no acute issues for general practitioner in the interim.

My appt with Dr Chan was so impressive I requested my care be transferred to her and to better coordinate my care with my husband's. Dr. Chan, while having a quiet, gentle manner, she immediately put me at ease and made me feel comfortable discussing my current health issue. She answered every question and made productive suggestions for speeding and maintaining my recovery.

This was my first visit with Dr. Chan as my new primary care. She reviewed my hx and rx. She was very easy to talk to and understand her explanations. She did not make me fill rushed at all. I'm very happy to have her as my new doctor.

Wonderful provider Dr Chan!!

I had two different people with several appointments changed by you. Previously you promised I would be getting a Physician not just a practitioner. My last visit I saw a Doctor who was fantastic. I requested she become my primary. Filling out the survey was difficult because of the changes In the last visits by Providence. The last visit by Dr. Chan was awesome. The level of service was exemplary by all standards especially the followup and explanations. I wish her to become my Physician of record..
