About Tracy Bazan
Tracy Bazan, M.D., earned her medical degree from New York Medical College and completed her internship, residency and fellowship at Oregon Health & Science University. She is board certified in neurology. Dr. Bazan's practices focuses on neuromuscular disorders. She joined Providence as a general neurologist in 2019 before transitioning to Providence ALS Center of Excellence in 2020.

Dr. Bazan was warm, professional, and a good listener. She helped me review all my symptoms and come up with a diagnosis that makes sense, and a treatment plan. She was very patient and responsive.

Dr. Bazan thoroughly explained the EMG/NCS results and ruled out progressive large-fiber nerve disease, which was reassuring. We also had a thoughtful discussion about small-fiber nerve biopsy and why it was unlikely to change treatment. She recommended blood work and gabapentin for symptom management. After the visit, I would have appreciated clearer guidance on additional non-medication options or referrals (e.g., mechanical or foot-specific evaluation) to address ongoing pain and numbness.
