Our Approach
Our Doctors of Nursing Practice, pharmacist, social worker and office staff welcome recently discharged patients to assist with their transition from the hospital to home. Our goal is to help our patients have improved quality of life, gain understanding about their medical condition, and avoid a hospital readmission. We typically see patients within a couple days of discharge. Our appointment generally lasts 40-mins to one hour. This is a comprehensive appointment as we review diagnosis, medicines prescribed, order medical equipment if appropriate, we discuss advance care planning and assess other needs. We’re located next door to St. Jude Medical Center and easy to find.
