Patients with hypertension, congestive heart failure or type 2 diabetes can stay connected with their health care team between visits by enrolling in Providence’s Remote Patient Monitoring (RPM) program with Cadence.

Once enrolled, patients receive easy-to-use devices to take their vital signs. These readings are securely transmitted to the remote care team to monitor, assess and respond to in real time. In coordination with your Providence provider, the remote care team will proactively adjust medications and provide ongoing lifestyle education and coaching to ensure patients stay on track with their health goals.

Personalized care

Daily monitoring helps spot changes in a patient’s condition early, so treatments can be adjusted in a timely manner to prevent complications or emergency room visits.

Avoided hospital visits

Patients enrolled in Providence’s RPM program are less likely to need a visit to urgent care or the emergency room for their chronic condition.
