Martina S. Cordova, NPC

Martina Cordova is a Family nurse practitioner, and alum of Sonoma State University. Prior to providing advance practice care, Ms. Cordova was a nurse for 12 years in outpatient chronic care setting, she developed long term relationships, addressed individual needs, educated about chronic disease management and prevention, and provided life-saving medical care for chronic disease. Ms. Cordova's approach in her family practice is committed to building a collaborative and caring relationship with patients, providing a holistic approach that includes physical, mental and emotional well-being. Ms. Cordova will partner with patients to manage chronic disease, education and prevention of diseases for healthy and productive lives. Together we can build a healthy and productive community.
