Kathryn J. Schroeder, PharmD

Dr. Schroeder is a Board-Certified Ambulatory Care Clinical Pharmacy Specialist in Primary Care, providing collaborative, evidence-based medication management in partnership with patients and interdisciplinary healthcare team members. She earned her Doctor of Pharmacy (PharmD) degree at Ohio Northern University, where she also completed a minor in Medical Humanities. She went on to complete two years of postgraduate residency training in community-based and ambulatory care pharmacy at West Virginia University/Kroger Health and Providence St. Peter Family Medicine/Chehalis Family Medicine, developing advanced expertise in chronic disease management and patient-centered pharmacotherapy. Her clinical practice spans comprehensive medication management across a range of chronic conditions. She specializes in complex chronic disease optimization, with particular emphasis on diabetes and cardiometabolic health, as well as mental health medication management. Dr. Schroeder is committed to expanding access to comprehensive pharmacy services and advancing the role of pharmacists within team-based care models to improve patient outcomes, safety, and healthcare delivery.
