Providence Care Choices helps you manage your health care. It is for those with cancer, heart failure, COPD or HIV/AIDS who have traditional Medicare insurance.

We can answer questions for you or your family. We can also provide supportive, palliative care to improve your quality of life. Our goal is to increase your comfort and ease your way. Medicare pays all costs – there are no copays or deductibles. This project is designed to bring you peace of mind.

We want you to live the best, longest life possible. Now you can continue treatment while also getting coordinated, supportive care. In Oregon, this model of care is available only at Providence.

A Providence Care Choices Care Coordinator will work with you and your family. They will assess your needs and identify your goals. They will create an individual plan of care for you. This will focus on increasing comfort and improving your quality of life. Your Care Coordinator will also talk with your health care team. They will be an advocate for you. They can help you make decisions based on your personal values. They can refer you to other resources if needed.

Providence Care Choices visits may be in person, by telephone, or by video visit on a computer, tablet, or smartphone. Video visits are a simple, personal and effective way for you to meet with a member of our team. You can schedule them when it’s convenient for you. Plus, you don’t have to leave home for a video visit.

If you or your family have a question or concern and cannot reach your regular healthcare provider(s), you can call us 24 hours a day at 503-215-0100. In an emergency, you should always dial 911.

This support is in addition to care from your primary healthcare provider.

You can go to any doctor who accepts Medicare patients. You can use the Medicare Physician Compare tool to help you find a doctor. When you make an appointment, verify that they will accept new Medicare patients.
