Preregistration

When your physician schedules you for a test, procedure or admission at Providence Milwaukie Hospital, we will try to contact you prior to your scheduled visit to obtain current demographic and insurance information. If you are having a surgical procedure, you may also schedule an appointment to speak with a pre-admission nurse to obtain further information and have your questions answered about the procedure you will be having.

Preregistering prior to your visit will expedite your registration process when you come to the hospital.

Being Admitted

When you arrive for your services at Providence Milwaukie Hospital, you will be greeted by a registrar who will gather information, obtain your signature on consent forms and provide you with patient rights and responsibilities and advance directive information.

Advance directives

An advance directive is a form that allows you to indicate your medical treatment choices before the need arises. If you are ever unable to communicate your desire for medical treatment, an advance directive can speak for you. The registrar will offer you this information at pre-admission or admission. If you have completed an advance directive, please bring it with you at the time of your hospital admission. This document will be placed in your medical record.

Personal Belongings

Keep your valuables and other personal belongings secure by following these recommendations:

  • Keep no more than $10 in your room
  • Leave your wallet and credit cards at home, or have them placed in the hospital safe
  • Leave your jewelry and other valuables at home

What to Bring

  • Your insurance card (if you have not been preregistered)
  • Robe and slippers
  • Toilet articles (comb, brush, razor, toothbrush and toothpaste, etc.)
  • A few dollars for personal items (newspapers, etc.)
