Patient Information

Before your first appointment

• Please bring all medications and supplements to your first appointment to review with your provider.
• Please bring your insurance card.
• We will ask you to pay your co-pay at each visit.

Please complete the medical records transfer form to transfer personal medical records from another doctor or hospital to our office.
