At Diagnostic Imaging at Providence Chase Medical Complex, we offer convenient diagnostic imaging services, including X-rays. An X-ray is a test that uses special waves to take pictures of your organs, tissues and bones to help doctors check for any problems. When you visit us for an X-ray, you can trust that our team will conduct thorough diagnostic assessments with compassion and precision so we can effectively address your medical concerns.

We also provide on-site treatment for various conditions, including fractures (bone breaks).

Records Requests

For your convenience, we provide forms to request copies of your diagnostic images.

• Use the Imaging Records Release form if you’d like your diagnostic imaging data and reports released to another medical facility.
• Use the Imaging Records Pick-Up form to authorize someone else to pick up your diagnostic imaging data and reports.
