If you do not see your insurance listed above or if you have questions regarding your coverage, please contact your insurance carrier for clarification. The physicians of Columbia Surgical Specialists are considered specialty providers in otolaryngology and neurotology. Some insurance plans require a referral or preauthorization for services performed by a specialist. Please verify this with your insurance plan and have these requirements in place prior to your appointment.
Most insurance plans require patients to pay a portion of billed services through a co-payment. You will be asked to pay your co-payment at the time of service. A service charge of $10.00 may be assessed for unpaid co-payments.

Patients not covered by insurance should be prepared to pay a deposit of $150.00 at the time of service.
