Frequently Asked Questions
For your visit, please arrive 15 minutes before your schedule appointment to ensure enough time to check in with the receptionist and fill out any necessary paperwork.
Please bring the following to your appointment:
- Co-payment if required by your insurance plan
- Detailed history of symptoms and treatments, including prior doctors and surgeons
- Full list of medications with dosages, including over-the-counter supplements
- List of your allergies
- List of your questions or specific concerns
- Photo ID and insurance card
Some intake forms may be completed prior to your appointment using your MyChart account.
Generally, most insurance companies cover rehabilitation services. We recommend that you call the member services or benefits number listed on your insurance card to verify your coverage before you start therapy. If you do not carry insurance, self-pay (cash) payment plans are available.
To prepare for your physical therapy appointment, wear comfortable clothing and shoes suitable for physical activity. Also make sure we can examine the affected body region.
If you are receiving physical therapy after surgery, have your surgeon send surgical protocol information to your physical therapist or bring it with you to your first appointment.
If you cannot attend your initial appointment, we may cancel all future appointments until you contact us to reschedule your treatment series. Please let us know if you need to cancel at least 24 hours in advance.
Please share your personal treatment goals with your care team and let us know if there are any significant changes in your condition. Your participation helps us tailor your treatment plan to your needs.
We will make every attempt to schedule you with the therapist you prefer. However, there may be times when a different physical therapist or physical therapist assistant will treat you in order to accommodate your schedule. All of our physical therapists and physical therapist assistants are fully licensed and/or certified and are committed to delivering high-quality care.
