To validate the status of the facility contract, please contact the St. Mary Student Coordinator at PSMMCStudents@providence.org or Kadlec Student Coordinator at AcademicServices@kadlec.org.

Step 2

Initiate a student rotation request by completing the following form. This can be submitted by either the student or the academic representative. A SEWA Student representative will be in contact with you regarding your request via email.

Step 3

If approved, the student onboarding process will be initiated by Academic Services. Students will be emailed the student intake packet to complete online. Student health requirements and additional health and safety requirements will be emailed to school coordinators to be filled out by the school representative, signed and dated attesting to the accuracy of the information provided.
