“Heart Failure Nurse Navigator Program Interventions Decreases Heart Failure Readmission Rates” won the Award for Moderated Poster Presentation in Practice Improvement at the American Association of Heart Failure Nurse’s (AAHFN) national conference in Austin, TX, on June 29, 2019. The poster abstract was published in the November-December 2019 issue of the Journal of Cardiopulmonary and Acute Care: Heart and Lung.
