Thank you for the trust you’ve placed in us to provide your patients with exceptional home health care and hospice care. It’s a privilege to provide your patients with the best possible nursing care or therapy services in a timely manner.

We serve home health patients in western Washington and in the Spokane region, and we serve hospice patients in western Washington. We promise to respond to all hospice inquiries within one hour.

Referrals

Referrals to Providence Hospice and Home Care come from a variety of sources. Typically these include the person needing care, a family member, family physician, hospital personnel, friend or clergy.

In order to qualify for home health care, the services must be intermittent, provide a skilled need, and in most cases the patient must be homebound. All services must be ordered by a physician.

In order to qualify for hospice care, the home services must be ordered by a physician and the patient must have a documented terminal diagnosis with an expected prognosis of 6 months or less, if the disease takes its expected course.

Providence Hospice and Home Care does not provide private-duty or hourly services.

How to make a referral

For your convenience, you can download and fax referral information with our physician referral fax form. When you call or fax us with your patient referral, please provide the following information:

  • Patient name
  • Face sheet
  • Copy of current medication list
  • Recent H&P
  • Hospital discharge summary or recent office visit notes to describe the patient’s current clinical condition
  • Copy of physician’s order for hospice care
Eligibility

The information in this booklet may be helpful for determining hospice eligibility; however these are guidelines only. Please contact us at 800-221-8022 anytime for assistance with identifying your individual patient needs.

Our services are available to patients of all ages, once they meet the criteria for hospice eligibility. Patients with the following illnesses may be hospice appropriate:

  • Cancer
  • Severe birth defects
  • Stroke
  • Amyotrophic Lateral Sclerosis

How to make a referral

When you call or fax us with your patient referral, please provide the following information:

  • Patient name
  • Name of the primary physician
  • Description of the health problem or concern
  • Services ordered: RN, PT, OT, SLP or support services SW, HHA
  • Patient’s date of birth
  • Patient’s social security number and Medicare or insurance number
  • Patient’s address and phone number
  • Patient’s family contact/emergency phone numbers
  • Hospital or skilled nursing facility information
  • Patient’s history and physical exam results
  • Patient’s medication list
  • A signed physician’s order

Our team will conduct an in-home assessment after we receive your referral. Once the in-home visit is completed, our clinician will call or fax your to review the plan of care.

Eligibility

Patients must meet all of the following criteria to be eligible for home health:

  • A physician orders home health treatment for the patient.
  • Patient is homebound, as required by insurance coverage.
  • Patient has a medical necessity.
  • Patient requires skilled care (not custodial) care.
