In many cases, we can provide relief with nonsurgical treatments, including physical therapy. If you require orthopedic surgery, you get the benefit of a highly experienced team of fellowship-trained orthopedic and primary care surgeons. Together, they perform more than 4,700 orthopedic surgical procedures each year at our location alone.   • Insurance card(s)
  • Insurance copay if applicable
  • Medical/Surgical history
  • List of your medications, including prescription, over-the-counter and herbal supplements
  • Personal documentation regarding your injury or condition that includes information such as when it started, what makes it worse, what seems to help, etc.
  • Medical records pertaining to your injury or current condition, such as imaging studies (please bring the actual film CD and Report if at all possible) or lab work. If you’ve been seen by another provider, when possible, it’s best to contact them a week or more before your visit and request that they forward your records to us.
