Online Application
You must print it prior to submission. It will not be available to print after that. No worries! Just send Lauren an email and she can print it out for you.
Please log back on and verify that your email address is correct on your profile. This is how we will inform and communicate with you in the future concerning your application. If you do not receive an email asking you to create an Empower account within 48 hours of submitting an application, contact our office.
Yes, please keep track of this info! Your PIN number will only be used if you need to finish your application at a later time.
PCT, CNA, Phlebotomy Tech, EKG Tech, Respiratory Tech, Radiology Tech, etc. Please provide a copy of your certification in your application packet.
Please provide your DD 214 or service record inside your application packet.
Acknowledgement Form
Be honest! We request a Background Check on all students from the Texas Board of Nursing, so they will have the final determination in this. They may request a Petition of Declaratory Order if necessary, but this will not happen until after you have been contacted by us about your application status.
Healthcare Forms
At the time of application, you will NOT be required to have one. Once you reach tentative acceptance, you will be required to have one. The testing needs to be done in a specific time frame, so please do not get it until instructed to do so.
No, please make sure a healthcare provider fills out and signs the form specifically for CSON.
Follow the Instructions on Option #2 for a Titer blood draw.
Please fill out the Covid Declination form found on our website. Covenant Health requires that you have had a Covid-19 booster within the calendar year you are applying, or fill out a declination.
Yes! Just take the Immunization Verification form to Covenant caregiver health and they will fill it out for you.
Official Transcripts
Yes, just make sure that they stay sealed in an envelope from the Institution.
Parchment.com is a great option that is available for most colleges. High Schools typically choose to mail transcripts to us.
We accept transcripts that are no older than one semester (typically 6 months).
Entrance Exam
No, the entrance exam must be taken at CSON. We accept test scores that have been taken within the past year (at our school) of your submitted application.
Yes, absolutely. Please contact us prior to registering so we can ensure we have space.
We recommend getting a study guide or using a website such as NurseHub.com to study and take practice tests.
Other Questions
Yes, in that case you will need to request a Letter of Standing from your previous program and include that in your Application packet.

Expect to hear from us by the 30th day after the Application cycle closes. The status on your Empower account will change and you will receive an email from us.
No. But, we highly encourage you to obtain either of these certifications because they hold great value in preparing you with basic nursing skills and hands on experiences, for our program.
Unfortunately, CSON is not equipped to accept foreign students. Therefore, all applicants must be U.S. citizens or have established permanent residency.
