The Sub-Acute Center at Little Company of Mary San Pedro is devoted to the treatment and care of patients with unique needs like ventilator and tracheostomy dependency. Patients may receive sub-acute care through rehabilitation, home care or long-term care.

Through an interdisciplinary approach to patient care, doctors at the Sub-Acute Center are able to maintain a homelike environment for patients while attending to their physical and psychosocial needs.

Our skilled medical teams of physicians, nurses, therapists, dietitians and case managers work in partnership to treat sub-acute patients. They strive to create an environment that minimizes disease progression, helps stabilize patients and returns patients to their highest level of independence and function.

Medical conditions that may require highly specialized sub-acute care include:

  • Respiratory failure
  • Spinal cord injury
  • Cerebral vascular accident
  • Severe heart attack
  • Traumatic brain injury
  • Multiple sclerosis

Our comprehensive sub-acute care includes a full spectrum of specialized services for patients with complex medical needs. These services include:

  • Ventilator Management/Weaning

Medical conditions frequently requiring ventilator care include respiratory failure, spinal cord injury, cerebral vascular accident, S/P myocardial infarction, traumatic brain injury, and multiple sclerosis. Our skilled medical team of physicians, nurses, therapists, dietitians and case managers all work together to care for our sub acute patients. Our goal is to create an environment which will minimize further disease progress, aid in preserving the stability of the resident, and return the individual to the highest level of function or independence as possible.

Patients admitted into our Sub Acute Care Center must meet the Department of Health Services criteria. That criteria includes:

  • Patient's condition warrants 24-hour access to nursing care by a registered nurse
  • Patient must meet one of the following:
    • Patient has a tracheostomy and requires mechanical ventilation at least 50 percent of the day
    • Patient has a tracheostomy and requires suctioning and room air mist or oxygen and one of the treatment procedures listed below:
      • Total Parenteral Nutrition (TPN)
      • Inpatient physical, occupational, and/or speech therapy at least two hours per day, five days per week
      • Tube feeding (nasogastric or gastrostomy)
      • Inhalation/respiratory therapy treatments at least 4 times per 24-hour period (not self administered by resident)
      • Continuous or intermittent intravenous (IV) therapy (via peripheral or central line)
      • Continuous or intermittent intravenous (IV) therapy (via peripheral or central line)
      • Wound debridement, packing, and medicated irrigation with/without whirlpool therapy
  • Administration of any three of the six treatment procedures listed above

Some patients come for a short stay, rehabilitate and return home. Other patients make our Sub Acute Care Center their home.
