Transforming Episode Accountability Model (TEAM)

Starting January, 2026, various ministries across Providence are participating in the CMS mandatory TEAM model. Under this model, these ministries have the opportunity to collaborate with health care providers and suppliers who are engaged in TEAM activities, and who may furnish health care services to you during your episode of care. Providence can enter into financial arrangements with these collaborators to share extra money received from Medicare.

The following list includes health care providers and suppliers that have an active TEAM Collaborator Agreement with Providence in order to share in financial rewards in the Transforming Episode Accountability Model (TEAM):
