If you are a patient or visitor at Mission Hospital, then you can rest assured knowing that you have come to the correct place for advanced care. If this is your first hospital visit, you probably have many concerns regarding your care - such as what to expect, who to ask questions of, and what is the best way to participate in your recovery. This information should answer many of your questions and make you feel more comfortable about your hospitalization.

Our team of nurses, doctors, technologists, technicians, therapists, and support staff all share in the tradition of caring. Together, we are dedicated to providing skills and services necessary to give you excellent, respectful care.

In order to make your visit as efficient as possible, we encourage patients to pre-register online for Labor & Delivery and Surgery services. Once registered, patients can be assured our professionals will be prepared for their visit, enabling them to complete their visit and checkout without delay. For insurance verification purposes, it is recommended that this form be submitted at least three days prior to your expected admitting date.

All patients need to stop by their designated registration area to complete the registration process. Patients are expected to pay all deductibles, coinsurance or co-payments at the time of service.

Pre-registration via phone 

You can call Mission Main Registration at 949-364-1400 ext. 1810 (Mission Viejo) or 949-499-1311 ext. 7167 (Laguna Beach) and ask one of our registrars to pre-register you over the phone. If a registrar is unavailable, please leave a voicemail and your call will be promptly returned.

Pre-registration via fax

Download and complete the Adobe Acrobat Online Pre-Admission Form and fax it to Mission Main Registration at 949-365-2306. For insurance verification purposes, it is recommended that you fax the registration form at least three days prior to your expected admitting date.

Channel 76 - Mission Health Channel provides patients with original, award-winning TV programs developed by experts. Patients can view a wide range of health topics, including important wellness and recovery information in an accessible and empowering way.

Patient education videos

Mission Hospital provides an online library of patient and family education videos that are integrated into the education and care we provide our patients and their family members.

A few benefits of these patient education videos are:

Mission Hospital is committed to providing interpreter services in order to maintain efficient and adequate communications with patients and families in their primary language. This includes services for patients and families who use sign language or Telecommunication Devices for the Deaf (TDD) to communicate.

Interpreter Services is available for a wide variety of languages. Sign Language services are also offered. Please contact your nurse or call 949-364-1400, ext. 4721.

Cancer support

  • A Step At A Time - A support group for women coping with breast cancer. The Nurse Navigator facilitates the group. Meets the second and fourth Monday of every month, 6 - 8 p.m. Held at Mission Women's Wellness Center classroom (171c).
  • A Place for Kids - For children and adolescents who have experienced the loss of a parent. Meets the first and third Mondays of every month, from Sept - May, 6 - 7 p.m. Held at Mission Conference Center.
  • Needle While You Work - Join other knitters in making hats and blankets for women going through chemotherapy. Or knot scarves for fundraising efforts during Breast Cancer Awareness Month. Meets the first Monday of every month, 10 - 11 a.m. at Mission Women's Wellness Center.
  • Our Journey Together - A support group that specifically addresses the needs of women who have recurrent and/or metastatic breast cancer. Meets the second and fourth Tuesdays of every month, 11:30 a.m. - 1 p.m. at Mission Wellness Center conference room.

Chemical dependency

Held on our Laguna Beach campus, these programs are for individuals in recovery and their families. Please call 866-633-6787 for exact locations.

Cardiopulmonary support

  • Mended Hearts - A volunteer education/support group consisting of people who have heart disease, their spouses, family members, medical professionals and other interested individuals. This group is facilitated by a cardiac rehab professional. Newsletters are published each month and an educational program is presented at each meeting. Meets the 3rd Wednesday of each month at 7 p.m. in the Mission Conference Center located at 26726 Crown Valley Parkway.
  • A Gift of Heart - An Organ Transplant & Donor Support Group - The Gift of Heart is an organ transplant and donor support group for patients and families of those who have received, are waiting for or who’s loved one has donated an organ. It provides a place to share the miracle and challenges that occur during this process. This group is facilitated by a registered nurse. Meets the 4th Monday of every other month at 7 p.m. in the Mission Conference Center located at 26726 Crown Valley Parkway. For more information call the CardioPulmonary Rehab Center at 949-364-7755.
  • PREP / Pulmonary Support Group - This is an educational/support group consisting of patients who have pulmonary disease, their spouses, family members, and other interested individuals. This group provides an opportunity for all individuals to ask questions, relate concerns and share common experiences with each other. The group is facilitated by the PREP coordinators. Newsletters are published each month and an educational program is presented at each meeting. Meets the 2nd Thursday of each month at 12 p.m. in the Mission Conference Center located at 26726 Crown Valley Parkway. For more information call the CardioPulmonary Rehab Center at 949-364-2106.
  • Healthy Heart and Lifestyle Series - This is a series of educational classes designed to help you learn the basics for developing a healthy lifestyle. Topics addressed include modifying behaviors, how the heart works, risk factor modification, stress management, exercise/activity guidelines and nutritional information. The series is held in the CardiaoPulmonary Rehab Center, 26732 Crown Valley Parkway, Suite 281, Mission Viejo, CA 92621.   • Heart Rhythms and Device Series - This is a series designed to help you learn more about your heart health related to an abnormal heart rhythm (such as Atrial Fibrillation) or an implanted device such as a pacemaker or internal cardiac defibrillator (ICD). Everyone is welcome to this fun, informal support group where you can express concerns with other people who have similar conditions. Meets the second Tuesday of every other month from 6 to 7 p.m. at the Mission Conference Center located at 26726 Crown Valley Parkway. For more information call the Cath Lab at 949-364-4880.


  • Diabetes Overview - This is a great class for individuals who have just been diagnosed with diabetes, as well as those who would like to learn how to better manage diabetes. The basic information can used to form a strong foundation for other classes.
  • Eating Well with Diabetes - This class teaches individuals how to control their weight and blood sugars, as well as plan and prepare healthy meals. The differences between carbohydrates, fats, and proteins will also be reviewed during the class time.
  • Diabetes Medications - In this class, a pharmacist will explain many of the medications that are frequently used to treat diabetes and other common co-existing conditions that individuals may be dealing with in conjunction with diabetes.
  • Fitness with Diabetes - Our exercise physiologist will discuss the role of exercise within managing diabetes. Exercise handouts will be provided, including options for individuals who have limited mobility or need low impact exercises.
  • Living with Diabetes - This class will include a discussion on how to effectively manage stress and anxiety, social situations and parties, and even how to work through the ups and downs that diabetes can bring.
  • The Connection between Diabetes and Sexual Function - Did you know that men with diabetes are at a high risk for developing erectile dysfunction (ED)? This class will discuss ED treatment options that are available. We encourage you to bring your spouse or partner to the seminar.

Free lectures & support group

In addition to the diabetes education series, educational lectures are offered throughout the year on special topics relating to diabetes. Lectures are free to anyone interested in learning more. Please call us if you would like to find out additional information on these lecturers and support groups, or if you would like to pre-register.

Schedule support from the Camino Health Center diabetes van

Like Camino Health Center, the van is dedicated to caring for medically underserved individuals in south Orange County. Services available on the diabetes van include: treatment by a physician, glucose screenings, blood pressure readings, and health/diabetes education. Call 949-240-2030 for more information and van scheduling.

  • Breastfeeding Support Group - Every Wednesday, 1 p.m. Join other mothers with babies of all ages, to discuss breast feeding joys and challenges, returning to work, nutrition and other topics. Hands-on help is not available during these meetings. Held at Mission Women's Wellness Center classroom 171c.
  • Postpartum Depression Support Group - This ongoing support group is for moms having unexpected feelings of anxiety, sadness isolation and depression following birth. Advanced registration is not required. Babies welcome. The postpartum depression support group is held Tuesdays, 10-11 a.m. at Mission Women's Wellness Center classroom 171c.

Pulmonary rehabilitation education program

As part of the Pulmonary Rehabilitation Education Program, we strongly encourage patients' families to participate in the program to become acquainted with the daily routine of pulmonary-afflicted individuals. This helps them learn how to overcome the obstacles impairing a patient's quality of life. Family members are also invited to attend a support group, which offers the opportunity to build relationships with other families managing the same challenges.

  • PREP Support Group - Patients with chronic lung disease and their families. Second Thursday of every month, 12:30 - 2 p.m. For more info please call 949-365-2106.

Mission Hospital offers a wide variety of inpatient, outpatient, home care and community services for seniors.

Senior transportation services

Through a partnership with Age Well Senior Services, Mission Hospital provides free non-emergency transportation to seniors receiving services at Mission Hospital. Through the senior transportation program, nearly 3,000 rides are provided each year to both Mission Hospital campuses. Participant and service criteria are:

  • Must be at least 60 years of age
  • Must reside within the cities of Aliso Viejo, Capistrano Beach, Dana Point, Ladera Ranch, Laguna Beach, Laguna Hills, Laguna Niguel, Laguna Woods, Lake Forest, Mission Viejo, Rancho Santa Margarita, San Clemente and San Juan Capistrano
  • All participants must register by phone prior to utilizing this service
  • Transportation will be provided to and from Mission Hospital, as well as between hospital campuses
  • Rides must be scheduled at least 24 hours prior to medical appointment
  • Transportation is provided on a first come first serve basis
  • Care attendant may ride with participant
  • Not for discharged patients or patients under anesthesia
  • Transportation hours are Monday through Saturday, 7 a.m. – 6 p.m. and is not available on major holidays.

Center for Rehab/Sports/Wellness

We understand how overwhelming Medicare can be. We know that you're flooded with information, which can make it difficult to know what to do and when to do it. Whether you're just turning 65, have been on Medicare for some time, or have Medi-Cal and Medicare, we can help. Butterfly Organ, Tissue and Eye Memorial dedication

In November 2016, Mission Hospital dedicated an Organ, Tissue and Eye Memorial in honor of those who gave their life to save another. The dedication was attended by 250 families and friends of donors whose names are honored on a plaque on the wall behind a beautiful butterfly sculpture named “Lifted Spirits.” Mission Hospital continually honors donors by holding an annual memorial event for the families and friends of recent donors. This is a note received from a mother of one of the honored donors.

“It was such a beautiful ceremony. I don't attend to really anything because it's really hard for me. I'm so glad I made the decision to go. I now see things differently. The man that made his speech about getting his liver made me see things different. How he appreciated his life and not taking anything for granted. I always question my decision, now I know I made the right one. I know there are a couple of girls getting a second chance because of my daughter. They have a second chance of life. And part of my baby lives on. Thank you for honoring our loves one, may God bless you!!!” 

Butterfly Organ, Tissue and Eye Memorial at Mission Hospital

At Providence, we support an individual’s right to choose the care they want. Our Ethical Directives of Catholic Healthcare call on us to respect the dignity of each person.

Advance Care Planning (ACP) encourages patients and families to talk about and document their care preferences in advance to ensure that the care they receive is aligned with their goals, values, and priorities. We have provided resources for you and your family to help you have the conversation and document your wishes in writing.

An advance health care directive lets your physician, family and friends know your healthcare preferences, including the types of special treatment you want or do not want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation. By considering your options early, you can ensure the quality of life that is important to you and avoid having your family make critical medical care decisions for you under stress or in emotional turmoil.

How to get started

The best time to start the conversation about the kind of care you’d want if you were in an accident or became seriously ill is now. The Institute for Human Caring is dedicated to engaging the community and health care providers in conversations about what matters and ensuring that our patient’s care preferences are honored. We can help you think about the care you’d want, talk to your loved ones about your decisions, choose your advocate and complete an advance directive.

Start the Conversation with these Four Steps:

  • THINK - about your values, goals and care preferences if you were to become seriously ill
  • TALK - to your loved ones about these care preferences
  • CHOOSE - someone to speak for you if you can’t speak for yourself
  • COMPLETE - an Advance Directive

Five Wishes

Five Wishes serves as an Advance Directive and is a legally-valid tool available for your use. Five Wishes helps ensure your wishes, and those of your loved ones, will be respected-even if you cannot speak for yourself.

POLST (Physician Order for Life Sustaining Treatment)

Physician Orders for Life-Sustaining Treatment (POLST) is a physician order that outlines a plan of care reflecting a patient’s wishes concerning care at life’s end. The POLST form is voluntary and is intended to:

  • Help you and your patient discuss and develop plans to reflect his or her wishes
  • Assist physicians, nurses, healthcare facilities, and emergency personnel in honoring a person’s wishes for life-sustaining treatment
