You can also call your dedicated phone line at 855-346-5057 to reach a Member Advocate. They’ll help you find a Providence primary care provider that’s right for you.
Transition Your Care
If you’re currently seeing a provider who is not in the Connected Care with Providence network, we can help you transition your care in a seamless way. We’ll assist you in completing the Transition of Care form and other necessary paperwork. If you are currently expecting a new little one, living with a chronic condition or receiving treatment for an illness, we’re here to make sure you get what you need during this process. Let us know. We can help you:
- Find a specialist
- Transfer your prescription medication
- Get the necessary authorizations for care
- Schedule medical procedures
- Obtain medical equipment necessary to your care
- Connect with your Care Navigation Team for ongoing support
- Submit your Continuity of Care and Transition of Care Request Form to request continuation of ongoing medical services
