Access to Care: Emergency Department Community Health Workers

The Access to Care Program in the South Bay and San Fernando Valley employs Community Health Workers in hospital emergency rooms to assist patients with:

• Applying for immediate medical health insurance coverage;
• Scheduling follow-up appointments to a local community clinic that can act as their primary care medical home; 
• Access to free imaging services to patients who qualify (San Fernando Valley only); and
• Navigating community resources.
