Patient Information
We ask that you arrive 15 minutes before your scheduled appointment to check in and complete any necessary paperwork. To maintain the schedule for all of our patients, we may need to reschedule your visit if you do not arrive on time.
Please bring the following:
- Copayment, if required by your insurance plan
- Detailed history of your symptoms and any previous treatments you've had
- Full list of the medications you take, including prescription, over-the-counter and supplements, along with their dosages
- Insurance card
- List of questions you have
- Photo ID
Please be aware we also reserve the right to dismiss you from our practice if you miss more than three appointments within one year.
