Anticoagulant medications, or blood thinners, help keep your blood flowing smoothly. They work to manage clotting after a heart attack or other illness. If you take anticoagulants, you’ll require close monitoring to avoid serious complications such as bleeding or stroke.
The pharmacists at Providence Holy Family Anticoagulation Clinics manage therapies that include warfarin (Coumadin®) and many newer oral or injectable anticoagulant drugs. We work with your doctor to provide lab testing, medication prescription and management and more at convenient outpatient clinics. Throughout your care, your health and safety are our top priority.
We provide many services through our outpatient clinics. In some instances, your care plan may also include home visits from nursing staff or a home monitoring program. Home monitoring for Anticoagulation services includes a simple fingerstick test you or a caregiver can do at home for instant results. The patient/caregiver will then report your results to the clinic to save time and travel needs. We provide other monitoring services that will help with medication safety and save time for the patient, as well as provide access to a knowledgeable care team that specializes in your medical condition.
At our Anticoagulation and Pharmacotherapy clinics, we offer complete care with specialized knowledge of medications from established drugs like warfarin to the newest oral and injectable therapies.
Services at our Anticoagulation and Pharmacotherapy clinics include:

- Prompt protime INR fingerstick test results at the time of visit
- Specialty-medication care for certain autoimmune and other conditions (select locations)
Providence pharmacists work with your doctor to test, review and evaluate your condition, including medications and diet. If you have or develop anemia, this means your blood is not delivering enough oxygen to the rest of your body.
Our clinic services include:
- Communicating with your doctors to prescribe and adjust your medications to treat and prevent anemia
- Individualized education about your condition and care
- Laboratory screening and medication review
- Maintaining a complete and accurate personal medication record
- Working with your doctor to ensure that you’re taking the most appropriate and affordable medication
Bring all your medications and supplements that you are taking and information about your health history. If you have difficulty understanding medical instructions or directions, please bring a family member or caregiver who can help you.
Our appointments are mostly for information and medication management. At some appointments, we will do a fingerstick, blood pressure check and occasionally an injection.
Most appointments take about 15 minutes, but for more complex conditions, they can last up to an hour. Wait times are generally 15 minutes after check-in.
Most insurances cover our service. If yours does not provide coverage, we will notify you in advance when possible.
You can park in our lot, just outside of the clinic. A wheelchair ramp is there if you need it.
