Daisy AwardThe DAISY Award was established by the DAISY Foundation in memory of J. Patrick Barnes who died at age 33 of ITP, an auto-immune disease. The Barnes family was awestruck by the clinical skills, caring and compassion of the nurses who cared for him. They created this national award to say thank you to nurses everywhere.

By filling out this form, you are nominating a nurse as a deserving recipient of the DAISY Award. This nurse's clinical skill and especially her/his compassionate care exemplify the kind of nurse that our patients, their families, and our staff recognize as an outstanding role model. The Needed, Acknowledged and Celebrated (NAC) Award recognizes other caregivers who support our patients, doctors and nurses and are integral in providing quality patient/family-centered care. The NAC award is presented bi-monthly to an employee whose skill and compassionate care exemplify the kind of assistance that can be recognized as an outstanding role model.
