One of our main goals at Community Outreach is to promote wellness and disease prevention, and to help individuals with chronic conditions better manage their own health needs. We promote independence in self-management of chronic conditions through home or office visits by nurses and social workers offering comprehensive support and care management services. Specifically, we provide assistance for the vulnerable and low-income populations.
  • Disease education (including coping and decision-making)
  • Medication compliance and adherence
  • Benefits, financial and community resources
  • Limited social or family support
  • Home safety concerns
  • Over- or under-utilization of medical services

Qualifications

Low-income, uninsured or underinsured clients who have chronic medical conditions and would benefit from case management services. For example individuals with:

  • Heart disease
  • Lung disease
  • Cancer
  • HIV/AIDS
  • Diabetes
  • Other chronic conditions

Personal assistance for people living with cancer

The CARE Network's Case Management program supports and assists low income residents of Napa County who have cancer and are under-insured or uninsured. We are a specialized team of health care professionals and client advocates dedicated to finding what you need to continue living well. Our services are free of charge, and are provided in English and Spanish.

Our team includes registered nurses and licensed social workers available to meet with you or come to your home to:

  • Simplify your health care by coordinating various medical services
  • Educate you and your family or caregivers about your health condition and necessary precautions
  • Explain nutrition and diet
  • Complete a medical and psychosocial assessment
  • Consult with you on managing your medications
  • Serve as a liaison between you and your doctor
  • Help you access health benefits
  • Assist with community resources and referrals
  • Provide emotional support for you and your family
  • Cancer support groups available

The CARE Network's Congestive Heart Failure (CHF) Case Management program supports and assists low income residents of Napa County who have CHF and are under-insured or uninsured. We are a specialized team of health care professionals and client advocates dedicated to working with you to determine your needs and helping you to identify resources which will meet those needs. Our services are free for eligible participants, and are provided in English and Spanish. A physician referral is required.

Our team includes registered nurses, licensed social workers and community care aides available to meet with you or come to your home to:

  • Educate you and your family or caregivers about Congestive Heart Failure
  • Enhance your ability to manage your health conditions at home
  • Consult with you on your medications
  • Simplify your health care by coordinating various medical services
  • Provide psychosocial support for you, your family, or your caregiver
  • Provide community resource guidance and assistance
  • Promote your independence
    Improve your overall health and quality of life

Personal assistance for people living with HIV/AIDS

The CARE Network's Case Management program supports and assists Napa County residents who are HIV positive or living with AIDS. We are a specialized team of health care professionals and client advocates dedicated to finding what you need to continue living well. Our services are free of charge, and are provided in English and Spanish.

Our case management team includes registered nurses and social workers available to meet with you or come to your home to:

  • Advocate with health care providers for early intervention
  • Educate you and your family or caregivers about your health condition and necessary precautions
  • Explain nutrition and diet
  • Provide medical and psychosocial case management and support
  • Serve as a liaison between you and your HIV / AIDS Physician Support as needed
  • Help you access health and social services resources and referrals
