CARE Network is a mission driven no cost program serving vulnerable individuals through intensive, community-based medical and psychosocial care management. Our holistic approach provides care to those who need it the most.
Who We Are
Queen of the Valley’s CARE Network nurse care managers, social workers, community health workers and behavior health therapists can support you in taking charge of your healthcare needs.
Who Qualifies
Services are provided to Napa County residents with complex medical and/or Socio-economic needs, including the following:
- One or multiple chronic conditions
- Lacking basic needs
- Homelessness and housing instability
- Mental Health challenges
- Substance use disorders
- Inadequate financial resources
- Uninsured and Under insured
- Limited Social Supports
- Language/health literacy barriers

What We Can Help With
Encouraging the self-management of Complex or Acute Health conditions by providing:
- Disease and Health coaching
- Complex Care coordination for both primary and specialty care
- Medication adherence support
- Connecting to a primary care provider
- Assistance with appropriately accessing medical services
- Assistance with navigating through medical resources Encouraging self-sufficiency by providing:
- Assistance with basic needs
- Access and connection to community resources
- Access to local, state and federal benefits
- Assistance with navigating through social services
- Help in building support systems
- Application assistance for Social Service Programs
